Registration Information

Pre-registration for the ASCI/AAP Joint Meeting closed on April 11, 2013, however, you may register at the meeting. Additional registrations will be accepted at the meeting beginning Friday, April 26.

To expedite the registration process, download our Onsite Registration Form and bring a completed copy to the Registration Desk located on the 2nd floor of the Fairmont Chicago Hotel, during the following times:

Friday, April 26	7:00 a.m. – 7:00 p.m.
Saturday, April 27	7:00 a.m. – 5:00 p.m.
Sunday, April 28	7:00 a.m. – 10:00 a.m.

Confirmation Letters
The Joint Meeting’s Web-based registration system acknowledges online registrations immediately via e-mail. You will receive a confirmation/receipt to the e-mail address provided on the registration form shortly after your registration is complete. The Joint Meeting will confirm registrations received by fax and mail within 14 days of receipt via e-mail as well. Note: The Joint Meeting is unable to confirm registration earlier than two weeks after the submission date if you do not provide an e-mail address. If you do not receive confirmation after two weeks, e-mail Kim Santos at Joint Meeting headquarters.

Payment Instructions
Registrations must include payment by check (drawn on a U.S. bank), credit card or bank transfer of funds in U.S. dollars. Wire transfers within North America will incur a $30 fee and wire transfers outside of North America will incur a $45 fee. The Joint Meeting cannot accept registrations by telephone. The Joint Meeting will not accept registrations without full payment. Caution: If you submit your registration form more than once, it may result in a duplicate charge to your credit card. Send your registration using only one method of payment.

Cancellation Policy
Notification of cancellation must be submitted to Joint Meeting headquarters in writing. A $50 cancellation fee will apply to cancellations received by April 11, 2013. Cancellations received after April 11, 2013, will be subject to a $100 cancellation fee. No refunds will be given after April 18, 2013.
